Background

Dr. Meskill is board certified in both neurology and sleep medicine, and he is the founder and CEO of Tricoastal Narcolepsy and Sleep Disorders Center. The "Tricoastal" moniker references his background: he completed neurology residency on the East Coast at Long Island Jewish Medical Center – where he served as chief resident, sleep fellowship on the West Coast at Stanford University, and he now practices sleep medicine and neurology on the Gulf Coast in the greater Houston, Texas area. In the sleep world, he is known for expertise in the treatment of narcolepsy, obstructive sleep apnea (OSA), and insomnia. He is published in peer-reviewed journals and texts, as well as online media. He has served as principal investigator in clinical research trials focused on narcolepsy and OSA. Additionally, he regularly serves on narcolepsy treatment advisory boards for Jazz Pharmaceuticals and Harmony Biosciences. He founded A.W.A.K.E. Greater Houston, a support group for individuals afflicted with narcolepsy and other sleep disorders that works in partnership with the Narcolepsy Network and the American Sleep Apnea Association. He has been recognized by Texas Monthly magazine as one of Texas’s Super Doctors Rising Stars (2018-20) and as a Top Doctor (2021), as well as one of Houston’s Top Doctors by H-Texas magazine (2015).


Narcolepsy and Other Disorders of Central Hypersomnia

Dr. Meskill’s professional passion is to improve the lives of patients suffering from disorders of central hypersomnia, such as narcolepsy and idiopathic hypersomnia. From 2019-20, he was the principal investigator of the Houston arm of the Pitolisant Expanded Access Clinical Evaluation (PEACE) program. During PEACE, he discovered that narcolepsy type 1 patients refractory to standard-of-care agents at that time had a 75 percent improvement in cataplexy with pitolisant treatment. Dr. Meskill was invited by Harmony Biosciences to travel to the F.D.A., and on December 12, 2019, he served as their U.S.-based clinical expert on pitolisant’s role in treating cataplexy. That meeting ultimately led to the F.D.A.’s decision to approve pitolisant (Wakix) to treat cataplexy in adults with narcolepsy.

Along with internationally renowned narcolepsy expert Dr. Kallweit, Dr. Meskill co-authored an expert opinion the treatment of narcolepsy. They called for better methods to recognizing and assessing cataplexy, rather than relying solely on quantifying events irrespective of severity or impact on quality of life.

In 2021, Dr. Meskill co-authored an analysis of randomized placebo-controlled trials using pitolisant to treat excessive daytime sleepiness and cataplexy in narcolepsy. That analysis showed a low number needed to treat for both sleepiness (3-5) and cataplexy (3-4), which supports the conclusion of robust efficacy of these agents.

Dr. Meskill twice (2019, 2021) has served as keynote speaker at the University of Texas’s Houston City-Wide Sleep Medicine Conference, where he discussed how to incorporate newer narcolepsy treatment modalities into clinical management strategies. From 2017-20, he was the Sleep Education Consortium’s (Houston) expert speaker on narcolepsy. He also was the guest lecturer for narcolepsy at the 2017 North Texas Nurse Practitioners annual continuing education conference in Dallas. In May 2022, he served as the faculty narcolepsy lecturer at the American Academy of Dental Sleep Medicine’s (AADSM) annual national meeting. In June 2023, he presented as an expert on idiopathic hypersomnia at the American Academy of Sleep Medicine’s (AASM) annual national meeting.


Obstructive Sleep Apnea (OSA) and Upper Airway Resistance Syndrome (UARS)

With Drs. Guilleminault and Arora, Dr. Meskill co-authored a meta-analysis on inspiratory flow limitation (IFL). IFL is a non-invasive measure of increased upper airway resistance during sleep, which can be utilized to diagnose upper airway resistance syndrome (UARS). Prior to utilizing nasal pressure transducer flow signal to measure IFL, patients with suspected UARS would undergo an invasive and uncomfortable procedure to have a Dobhoff feeding tube inserted intranasally and down into the mid-level of the esophagus. Called “esophageal manometry” (Pes), this tube then would remain in the esophagus for the duration of the overnight sleep study. With IFL monitoring, this no longer is necessary.

Dr. Meskill also co-authored a book chapter with Dr. Guilleminault on the increased risk of stroke in sleep disorders. In it, he noted that anywhere from 50-75 percent of individuals who have a stroke/TIA also have obstructive sleep apnea (OSA), but 80 percent of all individuals with OSA are undiagnosed. Likewise, having a stroke increases the risk of having OSA, and untreated OSA in stroke increases the four-year mortality rate by 21 percent.

From 2017-19, Dr. Meskill was the principal investigator of an investigation demonstrating that a simple physical exam maneuver could predict the likelihood of having moderate-to-severe OSA.

Over the past decade, he has been published numerous times on the Huffington Post’s Stanford Center for Sleep Sciences and Medicine sleep blog. In one instance, he challenged a New England Journal of Medicine’s results that CPAP does not prevent cardiovascular events in OSA, noting that the average CPAP use in the study was just 3.3 hours per night, invalidating the claims. In another, he advocated for Medicare reform. In two other instances, he highlighted how chronic insomnia and teeth grinding/clenching during sleep (sleep bruxism) can actually be signs of UARS. More recently, Dr. Meskill was featured in Men’s Health, where he talked about his own use of CPAP to treat sleep-disordered breathing. As he jokes with patients sometimes, “I’m not only the hair club president, but I’m also a client.

In October 2021, Dr. Meskill presented as part of a multidisciplinary team at the International Association of Orofacial Myology’s (IAOM) annual national meeting. The team of a speech pathologist (Kristie Gatto), sleep physician (Dr. Meskill), otolaryngologist (Dr. Bret Jensen), orthodontist (Dr. John Karotkin), and general dentist (Dr. Laura Lukin) presented an OSA case study to demonstrate the need for collaborative care to maximize patient outcome.

In December 2021, Dr. Meskill served as the sleep expert at the INGAA Foundation fatigue management workshop, where he addressed OSA and occupational dangers due to worker sleepiness. In May 2022, he served as the faculty UARS lecturer at the AADSM’s annual national meeting.

Since January 2023, Dr. Meskill has been a medical reviewer for SleepApnea.org.

Training

Out of high school, Dr. Meskill received an appointment to the United States Naval Academy, but ultimately he chose to attend the University of Notre Dame, where in 2003 he received his bachelor of arts (psychology). He served as Scholastic magazine’s sports editor from 2000-03, where he unofficially is recognized as the publication’s “all-time word count leader,” owing to his extensive writing throughout his tenure. In 2001, he won recognition from the Indiana Collegiate Press Association for the best sports story. In 2002, he was inducted into Alpha Epsilon Delta, the national premedical honor society.

 

From 2004-08, he attended medical school at S.U.N.Y. Downstate in Brooklyn, New York. In 2008, the American Academy of Neurology awarded him the medical student prize for excellence in neurology.

Dr. Meskill completed his internal medicine internship at Westchester Medical Center. He then completed his neurology residency at the Hofstra Northwell School of Medicine -- New York's largest integrated health system -- where he served as chief resident and was chosen by his peers to give the commencement address to the graduating class.

After residency, Dr. Meskill switched coasts to continue his training at Stanford University School of Medicine's Sleep Disorders fellowship program. Founded by William Dement, MD, Ph.D., Stanford's program is recognized internationally as the birthplace of the field and is considered the world's best academic sleep program. Dr. Meskill trained under many of the most renowned innovators in sleep medicine, such as Dr. Dement, considered the father of American sleep medicine; Emmanuel Mignot, MD, Ph.D., the chairman of the program and a world authority on narcolepsy; and Christian Guilleminault, MD, one of the world's most published authors in the field of sleep and a major contributor to the discovery of the physiologic mechanisms behind obstructive sleep apnea.
